Records Transfer

What’s the process for records release?

Your medical record belongs to you so a release of records can only be initiated by you.

• You may fill out a form with your new practitioner, who will mail it to me, or

• Print this release form and return it to the address specified on the form.
  
  

What will be included in transferred records?

In most cases, not your full chart but a summary of your health history, notes and labs from the last year of care.

Will there be a charge for transfer of records?

While there is no charge for the information outlined above, if more extensive records are needed there will be a cost of 20 cents/page for copying.
